Failed back surgery can be from scar tissue that forms near the nerve root.  This is called epidural fibrosis and this can be common after back surgery.  If recurrent pain comes on soon – 6-12 weeks post surgery – then scar tissue is the potential suspected cause.  Many times the patient will have pain relief right after surgery, and then pain just slowly comes back.  Back surgery is performed for many different reasons such as degenerative disc disease and herniated disc.

Also, fibrous adhesions of the nerve root can be considered as a cause of continuing back or leg pain after surgery.  Another reason for pain after back surgery is another disc herniation.  The patient may have had a disectomy and then has a sudden onset of pain from a herniation from the weakened disc that was operated on.

After surgery, if the evidence shows that back and or leg pain is due to scar tissue, the options are very limited for the patient.  In a period of the three months to year, medications such as Neurotin may be given to see what happens.  And exercise may help to free up the nerves.  If that doesn’t help, then the patient is limited to pain management for a life-time.
